Most med spa SEO fails because it optimizes for a search engine that no longer behaves the way the playbook assumes. Thin service pages stuffed with city keywords, a blog of filler posts nobody asked for, and zero schema: that package still gets sold every day, and it produces rankings reports instead of patients. What works now is narrower and harder to fake: real answers, real structure, and a site machines can verify.
The four failure patterns
- Thin service pages. Two hundred words of adjectives with the city name sprinkled in. Google stopped rewarding this years ago, and AI engines never rewarded it at all. There is nothing to cite.
- Content for keywords, not questions. A blog post titled "Botox Near Me" is written for a robot that no longer exists. Patients ask questions. Engines map questions to answers. Content that is neither gets read by no one.
- No machine layer. No schema, no landmarks, content rendered by JavaScript. The site may look fine and still be unreadable to the crawlers that decide.
- Rankings as the deliverable. Position three for a keyword nobody searches is a report, not a result. The deliverable is booked consults from search, or it is nothing.
What to do instead
- Answer-first pages. One page per real patient question, with the answer in the first paragraph. Cost, downtime, candidacy, comparisons. This is what both Google and AI engines lift.
- Deep service pages. Everything a patient would ask about the treatment on one page: process, pricing approach, recovery, proof, FAQ with markup.
- The machine layer. MedicalClinic schema, FAQPage markup, server-rendered content, clean headings. Boring, checkable, decisive.
- Local roots. Google Business Profile alignment, consistent NAP, reviews on a system. Search and maps feed each other.
How to hold any SEO work accountable
Three questions monthly: what questions do we now own the answer to, what does the AI visibility score say, and how many booked consults came from organic? If the report cannot connect to those, the work is decorative.

First find out where the leak is: run the audit. If the site converts poorly, rankings cannot pay. If the site converts and organic still books nothing, the rankings are for the wrong questions, and yes, that conversation is due.
How long does the answer-first approach take to work?
Technical fixes get read within weeks. Answer pages start earning citations and rankings over one to three months and compound after. Slower than an ad, permanent unlike one.
